To download and complete an application, first, click the "Download Application" button. The application file will either begin to download, or open in a new browser window, depending on the version of your browser. If the application downloaded, you may open the application and begin entering your information. If the application opened in a new window, simply right-click the application and select Save As. You can then open the application and begin entering your information.  When completed, make sure to save your document. You then can email the completed application to the email address listed on page 4 (be sure to attach the document to your email), or print the document and mail to the address also listed on page 4. Please note, due to the wide range of different computers and devices in use, not everyone will be able to fill in the application on their device.
